Use Vivacare's innovative Personal Update service to deliver automated, personalized self-care information to your patients regarding the medical topics you have recommended. Personal Update messages are sent during the following days, weeks or months when it matters most. By sending messages over time, important principles are reinforced and delivered when it is most relevant to the disease course. To provide your patients with Personal Updates, simply use your personalized "Information Prescription Pad" at the end of each appointment. Check off the recommended topics then hand the Information Prescription Pad to your patient. It takes all of five seconds. Your patients enroll for their own Personal Updates at your Vivacare Website then start to receive healthcare updates. (All personally identifiable health information remains secure) Patients with a specific medical condition will receive a "Disease Kit" that corresponds to that condition. Patients prescribed one or more medications will receive a "Medication Kit" that corresponds to those medications. Each Kit may include handouts, online videos, links to support groups, self-care advice, rebate coupons and other information. Pharmaceutical firms may enhance the Medication Kit regarding their products. This may include providing links to their product Websites or adding rebate coupons.
