Information We Collect About You
You will be required to submit Personal Information if you choose to register for the Vivacare Professional Service, make use of a mobile application, or access certain functionality on Vivacare Websites. Different types of Personal Information are requested by Vivacare depending on the type of content or service requested by the User.
“Personal Information” means information that we can use to identify or contact you, such as your name, address, telephone number, email address, or other identifier such as a professional membership ID.
You are responsible for ensuring the accuracy of the Personal Information you submit to Vivacare. Inaccurate information will affect your experience when accessing Vivacare Websites or using the Professional Service and our ability to contact you.
Access to Vivacare Professional Services is limited to medical professionals who register by entering Personal Information that enables Vivacare to create a Professional User account on their behalf. Information collected during registration for Professional Services includes:
- First and last name (required)
- Profession (physician, nurse, etc.) (required)
- Specialty (allergy, dermatology, etc.) (required)
- Organization (the hospital or clinic at which you provide care) (required)
- Location at which you provide care (required)
- Mailing address, if different from Location address
- Phone number (required)
- Fax number (optional)
- Email address (this becomes your username) (required)
- Password (required)
Information that can be entered either during registration or at any later date includes:
- Professional details
- Biography (html)
- Practice information
- Practice locations
- About the Practice
- Insurance accepted
- URL for the patient porta
The information you provide enables Vivacare to send you personalized emails and/or secure electronic messages and better tailor the content or service to meet your needs and the needs of your patients.
Communications to You
At registration and at various times when you use Vivacare Websites or Apps, you will be given the option of providing Vivacare with Personal Information in order to receive electronic or printed communications, such as Professional Service updates from Vivacare, newsletters from a medical professional, or messages from third parties.
By default, you will be opted in to receive newsletters from Vivacare. You may choose to opt out of receiving additional newsletters at any time by following the unsubscribe directions at the bottom of each email message.
Websites and Other Public Forums
Those who have registered for Professional Services may be able to use Vivacare to publish Personal Information and professional information to designated Vivacare Websites and to the mobile application.
Any information (including Personal Information) you share on a Vivacare Website or mobile App is by design open to the public. You should think carefully before posting any Personal Information in any public forum. What you post can be seen, disclosed to or collected by third parties and may be used by others in ways not possible to control or predict, including to contact you for unauthorized purposes. As with any public forum on any site, the information you post may be found by third-party search engines.
If you mistakenly post Personal Information to a Vivacare Website and would like it removed, you may send us an email to request that we remove it by contacting customer support through the Vivacare Website. In some cases, we may not be able to remove your Personal Information while still maintaining an account on your behalf.
Additional Information Collected
Due to internet transmission standards, when you visit the Vivacare website or mobile App, certain information is recorded. This information includes the URL of the site from which you came to Vivacare and the URL of the site you went to when you left Vivacare. Your computer’s operating system and type of web browser and your ISP or mobile carrier are also recorded. We use this information to analyze trends and help improve our services.
Vivacare may work with third parties to display content on some of the Vivacare Websites. These third parties may use their own Cookies or other online tracking technologies to monitor the usage of their content. These technologies are not used to display advertising.
Your Right to Access or Correct Information about You
You can correct, update or review Personal Information you have previously submitted by going to the Administration Portal, logging-in and making the desired change. You can also update any Personal Information you have submitted by contacting Vivacare Customer Support.
The Vivacare Services cannot provide value to you without using the personal information you enter during registration. If you choose not to share this information with Vivacare, you may delete your account and discontinue using the Services.
Uses of Personal Information
Vivacare is a service designed to help medical professionals provide valuable information to their patients. The information about yourself that you choose to share with us helps us provide relevant, current information to your patients on your behalf.
Other ways we use your personal information:
- To administer your account
- To provide you with access to particular tools and services
- To respond to any inquiries you send to us
- To send you administrative communications about Vivacare Websites or services
- To obtain your feedback about Vivacare Services
- Conduct research and measurement activities, including those described below.
Vivacare may combine Personal and Non-Personal Information collected by Vivacare about you, and may combine this information with information from external sources.
Sharing Information with Third Parties
The Vivacare Customer Support team has limited access to your Personal and Non-Personal Information as needed in the course of providing products or services to you.
Disclosures to Third Party Web Sites
Vivacare does not sell, rent or otherwise provide your personal information to third parties for marketing or other services.
Securing your Information
We use industry standard security measures to protect your Personal Information. However, despite Vivacare’s efforts to protect your Personal Information, there is always some risk that an unauthorized third party may find a way around our security systems or that transmissions of your information over the Internet may be intercepted.
The security of your Personal Information is important to us. We encrypt all Personal Information during transmission and when stored in our database. All Vivacare employee access to your data is on a “need to know” basis. We host all our data at Rackspace, an industry leading, secure cloud hosting provider. Rackspace infrastructure and facilities are regularly audited, and have achieved numerous certifications, including: ISO 27001/27002; PCI-DSS Level 1; SSAE16 SOC1, SOC2 and SOC3; Safe Harbor, and CPS.
You may remove your Personal Information or instruct us to remove it at any time, but you should be aware that residual information about you will remain in our backups for a period of time. We will also retain your Personal Information as necessary to comply with legal obligations, resolve disputes and enforce our agreements. All Personal Information about you that we retain for any reason will be stored using the same strict security practices described above.
Eligibility to use Vivacare Services
Vivacare Services are available to all U.S. residents over the age of 18. The Vivacare Professional Services are available to physicians and other medical professionals. You may be required to provide credentials that confirm your legal standing to provide medical care.
